Toronto West Stroke Network

The Toronto West Stroke Network is one of 11 regional networks established across the province to implement the Ontario Stroke System. The Ontario Stroke System is a Ministry of Health and Long Term Care initiative, with the goal to promote equitable access and improve outcomes for stroke survivors and their families by integrating best practice stroke care in the areas of prevention, emergency and acute care management, rehabilitation and life re-engagement into the community following stroke. The Toronto West Stroke Network represents a regional collaboration of partners in stroke care delivery in these areas that seeks to re-organize stroke care, promote system change and support professional education and public awareness in achieving this vision.
