Toronto Stoke Networks Steering Committee Annual Achievement Report for 2014-15

The Toronto Stroke Networks (TSNs) are pleased to highlight the many accomplishments achieved by the members of TSNs in this 2014-15 Annual Achievement Report. These accomplishments represent:

  • Sustainable growth toward our vision of Fewer Strokes Better Outcomes.
  • A transition toward refreshed strategic directions, goals and structures.
  • Continued alignment and responsiveness to the Ministry of Health and Long Term Care priorities and Health Quality Ontario’s Quality Based Procedures (QBP) for stroke.

Communiqué – Stroke Flow Guidelines Implementation

In December 2012, the Toronto Central LHIN (TCLHIN) established the MSK/Stroke Implementation Group to support implementation of best practices related to total joint replacement, hip fracture and stroke. One of the priorities of the group is to keep stakeholders informed and engaged throughout the project.

The purpose of this Communiqué is to provide an update on key activities supporting implementation of the recommended stroke flow guidelines. Subsequent updates will follow throughout 2013/14.
