An Online Orientation Module to Support Essential Professional Conversations

Essential Professional Conversations (EPCs) or ‘warm handovers’ foster enhanced communication, learning and meaningful cross-system collaboration between healthcare providers at times of transition. Verbal exchanges between healthcare providers across facilities can optimize the continuity of care for persons with stroke and their families, and in particular, for persons with complex needs after stroke.

 As a standard approach to support transitions within Toronto Stroke Networks, EPCs are being encouraged for all persons with stroke that are considered to have complex needs.  To support implementation of EPCs, a new online orientation module has been developed for all healthcare providers working in stroke care.  To access this new resource, please click on the following link:
