QBP Handbook for Acute and Post-Acute Stroke/TIA Care
Since April 2012, the Ministry of Health and Long-Term Care has shifted much of Ontario’s health care system funding away from its global funding allocation toward a funding model that is founded on payments for health care based on best clinical evidence-informed practices. Quality Based Funding (QBF) supports system capacity planning and quality improvement through directly linking funding to patient outcomes. QBF provides an incentive to health service providers to become more efficient in their patient management by adopting best practices that ensure Ontarians get the right care at the right time and in the right place. The newly released Quality-Based Procedures Handbook for Stroke (Acute and Post-Acute) will replace the previous QBP Handbook, and now encompasses post-acute episode of care 60 days after stroke onset.
