Thank you for registering for the Toronto Stroke Networks’ inaugural Community Forum. The objectives of this event are:
To educate community care providers of the existing Community Stroke Best Practices
To provide resources and tools that community providers will be able to utilize in their practice
This is a conference-style event, where you will have the opportunity to attend any or all of the 7 scheduled sessions. Please register for the one(s) you would like to attend, as a separate Zoom link will be sent for each session.
We recommend everyone registerfor the “Introduction to the Toronto Stroke Networks” session, where a Patient & Family Advisor will share their experience in receiving carein the community.
