The Toronto Stroke Networks (TSNs) lead and coordinate the adoption of stroke best practices across the system of care. In collaboration with regional partners, the TSNs have developed a sustainable Knowledge Translation (KT) strategy that supports cross-system collaboration and strengthens relationships and trust leading to system integration.
